Procedures for Scheduling Web Consultation
Items

《IVF》
1. IVF Free Consult： maximal twice free consulatation for 15 min :  will file the personal information 

2. IVF Enroll Consult：  Setting IVF Schedule and protocol NTD 3000/USD 100 ：                         
   Enrolled in IVF,  Set the IVF protocol, Drugs and Schedule for stimulation, Selection of Fresh ET/Freeze all/FET, methods of embryo selection, and PGT-A/NGS, ERA

    ► 1st generation of IVF, 2nd generation of IVF, 3rd generation of IVF

    ► Before the Zoom meeting of IVF Enroll Consultation, you can upload your medical information or file your related documents. After the Web consultation for PGT and IVF, we will schedule an IVF plan.

    ► IVF Enroll Consult (40 min limit) NTD 3000  《Key to pay》

      or you will receive invoice from IVFPRO LLC for USD

《Consulation for Surgery》: Web consoulation is only available for EMPRINT surgery
3. Surgery Free Consult：one free consulatation for 15 min :
4. Surgery Enrolled Consultation：You will be enrolled for surgery and we will have a review of our surgery plan and schedule,  NTD 3000/ USD 100 ：You will be enrolled into treatment protocol. 

《Endocrine/Slim Body》
5.  Endocrine/Slim Body Protocol and Consultation：
   one (10 min limit) Manderin-Speaking Video Meeting for NTD 1500/ USD 50 
   or one (40 min limit) English-Speaking Video Meeting for USD 100.
   Will complete the setting of treatment protocol and prescription for 2 months (cost of  
  drugs is not included).
